Hospice is designed as a team approach to health care that includes the services of a physician, nurse, hospice aide (CNA) and social worker. Many patients and families also choose to include additional team members such as a chaplain or spiritual counselor, bereavement counselor and volunteer.
The hospice care team offers a comprehensive care plan that quickly responds to the changing needs of both patient and family.
